Do you have experience in supporting vulnerable adults and empowering them to move forward with their journey?

Walking With The Wounded (WWTW) has an exciting opportunity for a Support Care Coordinator (SCC) to join a new project in partnership with the NHS and another third sector organisation.

We are looking for someone who can work independently as well as part of a small collaborative team who will empower ex-service personnel to regain their independence, thrive and contribute in our communities. This is a new role so you must be a self-starter with the motivation and the ability to get the job done, you will form part of a wider collaborative team which includes the Norfolk and Suffolk Foundation Trust and Outside the Wire. You will be home based with some travel around the region to build relationships in the community.

WWTW supports those who have served in the British Armed Forces and their families. We specialise in developing pathways to independence as well as employment with organisations who recognise and value the unique skills and qualities of our ex-service personnel.

We are looking for a self-starter with prior experience of working with vulnerable adults, an individual who can work independently to set priorities that accurately reflect the relative importance of role responsibilities.  Primarily you will be part of a small team of staff at WWTW operating in a much wider collaboration that includes the NHS, other service charities and local support services.  Your role will be to work with clients remotely, and at times intensely, to support by analysing individual needs to create a support plan that will break down barriers to a successful stabilisation and navigate the pathway of their MH journey.

In return, we will provide you with the opportunity to demonstrate and develop your skills and career as a Support Care Coordinator in a supportive, inclusive environment. In this role, no two days are the same and every day will bring new challenges. Internal training will be provided and we encourage our staff to develop their own personal and professional skills.
