Hours: Part time – 18.5 hours

Walking with the Wounded (WWTW) and the Armed Forces Outreach Service (AFOS) support those who have served in the Armed Forces and their families, empowering them to regain their independence, thrive and contribute to our communities.

In Partnership with the Armed Forces Outreach Service, we have been successful in securing funds from the Armed Forces Covenant Fund Trust for a joint project in the Northeast of England.

At its core, the Joint service will recognise that for some ex-servicemen and women, the route to support looks complex and often inaccessible. By providing a joint Care Coordination service, the programme will ensure these individuals are provided the breadth of care they need, as well as providing support to their families and carers by utilising the Armed Forces Outreach Service’s access to council services across Gateshead, Northumberland, Newcastle, and Durham.

Our Care Coordinator will have a small caseload of ex-servicemen and women and will also be the conduit to support from the AFOS team in the relevant area. Working with our local delivery partner at AFOS, you will make referrals for local support to the council based AFOS advisors. Your role will be crucial to the success of the project, and you will help advertise, promote and support the AFOS service. If you are forces friendly and have experience of welfare support with some knowledge of regional support services, please do get in touch.

Collaboration and teamwork are key; however, you must be someone who is able to work at times independently, particularly in managing your time and caseload, whilst recognising that you are one crucial element of a wider team of support who will need to work collaboratively to get the best outcome for clients. Experience of working with veterans would be beneficial although not essential.
