We have an incredible opportunity for two part time (17.50 hours per week) Case Managers to join our Team in the Welsh region. One based in North Wales, one based in South Wales.

About You

Do you have two or more years of experience working in the welfare or support sector with adults at risk, and a Level 3 Diploma in Welfare or other relevant discipline? Are you skilled in working in a multidiscipline environment supporting the delivery of case management? If you also have first-rate interpersonal and communications skills, are excellent in task management and can demonstrate high levels of integrity, confidentiality and discretion look no further. This is the role for you.

About the Role

This is a varied role, instrumental in delivering and co-ordinating the support provided to our wounded, injured and sick veterans. As part of our Case Management Team, you will hold the responsibility for the assessment of needs of individual Veteran, supporting them through their recovery journey using the Case Management framework. You will be responsible for supporting their specific Welfare needs which could include housing or debt. Within Case Management, you will co-ordinate the professional support for the veteran as part of the Multi-disciplinary Team that will surround that individual and their family. This will include Recovery Planning, goal setting, reviews of their progress into independent living, also the provision of high-quality information and guidance in relation to all aspects of their recovery journey.

We support our veterans to break down barriers, regain control of their lives; the gateway to the person they want to be. We work collaboratively alongside all our recovery services, working to deliver effective, timely and impactful case management support to our veterans and their families who are living with ongoing injuries or illness from their time in, or as a result of service.
