WWTW has been successful in securing funds from the Armed Forces Covenant Trust for a new project in the North East (Gateshead) of England. Northern Care Coordination Partnership (NCCP) has launch a new, impactful and innovative service that can be replicated across the UK.

At its core, the NCCP recognise that for some ex-servicemen and women, the route to care looks complex and often inaccessible. By providing an integrated care coordination service, the NCCP will ensure these individuals are provided the breadth of care they need, as well as providing support to their families and carers. NCCP will be an impactful and crucial service to a small cohort, which is highly ‘in need’.

Support Care Coordinators will have a small caseload of complex ex-servicemen and women. Working with up to ten local delivery partners, you will make referrals for specialist support, whilst retaining the overall caseworker responsibilities. Your role will be crucial to the success of the project. If you are an experienced caseworker with an understanding of working with more complex clients, we would like to hear from you.

You should fully appreciate the patience, resilience and self-motivation that are required to support a dispersed and diverse caseload of complex individuals. You must be someone who is able to work independently, particularly in managing your time and caseload, whilst recognising that you are one crucial element of a wider team of support who will need to work collaboratively to get the best outcome for clients. Experience of working with veterans would be beneficial although not essential.
