Location: Homebased with travel to multiple locations within Surrey, North Hampshire, Berkshire and South Buckinghamshire. 

Salary: £25,000

Hours: 37.5 hours per week (e.g. 9-5, half hour unpaid break)

Contract type: 12 Months fixed term (potential to extend subject to funding)

Walking With The Wounded ‘WWTW’ deliver employment, mental health, care coordination and volunteering programmes in collaboration with the NHS to get those who served, and their families, whether mentally, socially or physically wounded, back on their feet and making a positive contribution once more. The Veterans’ Places, Pathways and People programme (VPPP) is an Armed Forces Covenant Fund Trust (AFCFT) funded programme. Walking With the Wounded is one of several organisation members of the VPPP programme in the South East, with the Veterans Outreach Service operating as the lead. As our contribution to the South East offering, we will be employing a new Care Coordinator.

The successful candidate will ideally be in the Frimley/Farnborough or neighbouring areas to enable reach into Surrey, North Hampshire, Berkshire, and South Buckinghamshire.

WWTW’s Care Coordination service involves the assessment of needs for your client, the sourcing of appropriate support and the ongoing navigation of that support. You remain the central point of contact as you support your service user to address their challenges; which may include a lack of secure housing, debt, social isolation, relationship breakdowns, lack of education and training, welfare issues or substance misuse.

We are looking for a Care Coordinator who fully appreciate the patience, resilience and self-motivation that are required to support a dispersed and diverse caseload of complex individuals. You must be someone who is able to work independently, particularly in managing your time and caseload, whilst recognising that you are one crucial element of a wider team of support who will need to work collaboratively to get the best outcome for clients. We welcome applicants who have experience or specialise in housing, although it is not essential. Experience of working with veterans would be beneficial although again, not essential.

The Care Coordinator will have a caseload of complex ex-servicemen and women. Working with local delivery partners, you will make referrals for specialist support, whilst retaining the overall caseworker responsibilities. Your role will be crucial to the success of the service user’s outcome. If you are an experienced caseworker with an understanding of working with more complex clients, we would like to hear from you.

WWTW has continued to deliver Care Coordination during Covid19. Where possible support is delivered via remote means. Where face-to-face support is deemed necessary for the needs of the service user, WWTW take staff health and wellbeing very seriously and we are supporting our teams with safe face-to-face working.
