WWTW has been successful in securing funds from the Armed Forces Covenant Trust for a new project in the North East (Gateshead) and North West (Manchester) on England. Northern Care Coordination Partnership (NCCP) will launch (and trial) a new, impactful and innovative service that can be replicated across the UK.
At its core, the NCCP recognise that for some ex-servicemen and women, the route to care looks complex and often inaccessible. By providing an integrated care coordination service, the NCCP will ensure these individuals are provided the breadth of care they need, as well as providing support to their families and carers. NCCP will be an impactful and crucial service to a small cohort, which is highly ‘in need’.
You will manage a team of three Care Coordinators in Gateshead and Manchester, who will each have a caseload of complex ex-servicemen and women. Working with up to ten local delivery partners, the Care Coordinators will make referrals for specialist support, whilst retaining the overall caseworker responsibilities. Your role will be crucial to the success of the project and you should be adept at people management, project management, stakeholder management and working to targets. You will establish the steering group of delivery partners and manage those relationships at the highest level.
This is a key role for WWTW with a significant degree of responsibility. You must be an experienced project manager looking to take ownership of this new and exciting project. You must be someone who is able to galvanise and manage external stakeholders to ensure success of the project. You should be able to work independently, particularly in managing your time and priorities, to a set of targets. Experience of working with veterans would be beneficial although not essential. You must have experience of managing a team to a set of targets and delivering on those targets.
You will be based in Gateshead for three days per week and in Manchester two days per week.
